Hello there. I appreciate that you are looking for views, and I would be willing to support your survey (anything to make the life of a working mum easier!) but I am not sure whether you can expect someone to give their honest views when you also require them to give their name, position and the name and address of the company they work for. I think that you are more likely to get honest views if the survey is conducted on a more private basis than that. Just my opinion.
